This session provides an overview of PA Navigate, Pennsylvania’s coordinated referral platform designed to connect individuals with community-based services that address social determinants of health. Participants will learn how organizations can use PA Navigate to make and receive referrals, track outcomes, and streamline care coordination. A live demonstration will showcase key features and user functionality, offering a practical understanding of how PA Navigate supports collaborative community impact.
